Implant Placement in the Esthetic Zone for Peri-implantitis Prevention from Periodontic Viewpoint

Abstract
Implant-supported fixed prosthodontic restorations have been proven to be very predictable in recent studies. Especially in partially edentulous anteriors where esthetical consideration is of indispensable concern, implant treatment has enabled restorations without preparation of adjacent natural teeth. Natural and esthetic implant restorations allow high satisfaction for patients. However, in the course of time, many factors may lead to the development of peri-implantitis. In order to prevent and minimize peri-implant disease, meticulous plaque control and periodic supportive periodontal therapy are extremely crucial, starting at the time of the surgery until completion of the final restoration. The position of ideal implant placement has been changing. In the 1980s, implants were placed in the alveolar bone with sufficient volume. As the direction of the implant for ¡®bone-driven implant placement¡¯ had no consideration for prosthodontic restoration after completion of osseointegration, sometimes it was difficult to build restoration at all or unesthetic restorations were highly probable. In the 1990s, implants were placed in an ideal position for esthetic restorations after osseointegration independent of alveolar bone volume. Recent progress in implant related bone regeneration techniques has allowed preference for ¡®restoration-driven implant placement¡¯ for esthetic restorations. In the 2000s, not only esthetic restorations were produced by restoration-driven implant placement, but the concept of ¡®biologically-driven implant placement¡¯ was introduced. Implant placement considering the biologic width of peri-implant mucosa decreases the probability of peri-implantitis. Implants should be placed based on this concept and prevention of peri-implantitis is the most fundamental approach to maintaining esthetic restorations. Various surgical considerations are needed to prevent peri-implantitis with a good understanding of principles of periodontology.
